I've had a version of the same conversation approximately forty times in the last eighteen months. It goes like this.
A CTO or VP of Engineering reaches out. They've been tasked with "doing something with AI." They have a budget. They have engineers. They've run a few pilots. Nothing has shipped to production. They want to know what they're doing wrong.
The answer is almost always the same. And it has nothing to do with the model they're using.
The Real Reason AI Projects Fail
The most common failure mode in enterprise AI is not technical. It is organizational. Companies treat AI like they treated cloud migration in 2012 — as a technology problem to be solved by the technology team, handed off to the business when it's "ready."
But AI is not a technology problem. It is a judgment problem. And judgment cannot be delegated to a team that is isolated from the decisions being made.
Here's what I mean. When a company builds a RAG pipeline to answer customer service questions, the technology part — the embeddings, the retrieval, the LLM call — is the easy part. In my consulting work, I've seen three distinct patterns of failure. They're worth naming.
The Pilot Graveyard. The company has run twelve AI pilots in the last two years. None of them are in production. Each one was declared a success in the demo and then quietly shelved when it came time to integrate with real systems, real data, and real users. The pilots were optimized for impressiveness, not deployability. The lesson: never build a pilot you're not prepared to ship.
The Vendor Trap. The company has signed contracts with three AI vendors, each of whom promised a turnkey solution. Eighteen months later, they have three systems that don't talk to each other, a data governance nightmare, and a total cost of ownership that is three times the original estimate. The lesson: AI is not a product you buy. It is a capability you build.
The Model Fetish. The company is waiting for GPT-5, or the next Claude, or whatever model is rumored to be coming next. Their reasoning: the current models aren't good enough for their use case. This is almost never true. In 99% of cases, the current models are more than capable. The bottleneck is not the model. It is the data, the architecture, and the organizational will to ship. The lesson: the model you have today is good enough. Ship something.
What the Winners Are Doing
The companies that are actually winning with AI share a set of characteristics that have nothing to do with their technology stack.
They have a clear definition of "done." They know, before they start building, what success looks like — not in terms of model accuracy, but in terms of business outcomes. "Reduce customer service resolution time by 30%" is a definition of done. "Build a chatbot" is not.
They treat data as a first-class product. The companies that are furthest ahead in AI are not the ones with the best engineers. They are the ones with the cleanest, most accessible, most well-documented data. They invested in data infrastructure years before AI was a priority, and now that investment is paying compound interest.
They ship small and iterate fast. The winning pattern is not a six-month AI transformation project. It is a two-week sprint that ships something real, learns from real users, and informs the next sprint. Here is the question I ask every company I work with, and the answer tells me almost everything I need to know about their AI readiness:
"If your best AI engineer quit tomorrow, what would happen to your AI capabilities?"
If the answer is "they would stop," you don't have an AI capability. You have an AI dependency. The goal is to build AI into your systems, your processes, and your organizational knowledge so deeply that it survives the departure of any individual.
That's a high bar. Most companies are nowhere near it. But the ones that are working toward it — the ones that are documenting their AI systems, training their teams, and treating AI as infrastructure rather than magic — are the ones that will still be winning in five years.
What This Means for You
If you're a CTO or operator reading this, here's my honest advice.
Stop waiting for the perfect model. Stop running pilots that aren't designed to ship. Stop treating AI as a technology problem and start treating it as a business transformation. And if you don't have the internal capability to do this — if you're stuck in the pilot graveyard or the vendor trap — find someone who has shipped production AI before and can show you the path.
